How often is it reported?

A weekly report with immediate alerts for urgent matters is usual. What counts is not frequency but that an urgent finding reaches you before the deadline rather than in the next cycle.

Can monitoring not be automated?

Collecting yes, assessing only in part. A keyword finds the title of an initiative but not the point in article 14(3) where a definition captures your business model. Those are exactly the expensive ones.
